Photo & Media Release Consent Form

If you would like to share photos from your vacation, please consent below to Adventure Book Travel LLC using your images and upload your pictures. This is not required; however, we do love to show off our happy clients and are always appreciative of those who share their memories with us!

I consent & grant permission for Adventure Book Travel LLC to use my images I share with the agency as listed below in the release. I also agree that I have permission to share this/these photo(s) and that I'm consenting to the release for all parties in the photo(s).

PHOTO RELEASE

For good and valuable consideration, the receipt of which is hereby acknowledge, I, (named below), hereby grant Adventure Book Travel LLC permission to use my likeness in a photography in any and all of its publications, including but not limited to all of Adventure Book Travel LLC's printed and digital publications. I understand and agree that any photograph using my likeness will become property of Adventure Book Travel LLC and will not be returned.

I acknowledge that since my participation with Adventure Book Travel LLC is voluntary, I will receive no financial compensation.

I hereby irrevocably authorize Adventure Book Travel LLC to edit, alter, copy, exhibit, publish or distribute this photo for purposes of publicizing Adventure Book Travel LLC's programs or for any other related, lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph.

I hereby hold harmless and release and forever discharge Adventure Book Travel LLC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I am above the age of 18 years old and am signing this release on behalf of all family members that were booked on this trip with Adventure Book Travel LLC.

I represent I have carefully read and fully understand all the provisions of this release and am freely, knowingly and voluntarily entering in to this release.
